Pick up or Delivery Date

Billing Address

Shipping Address (If applicable)

Amount and Type of Specimens Needed:

Please Include any specific details. You may also email pictures.

Documents Needed to Complete Your Order:

1.) Application and Permit to Obtain Specimens

2.) Formal Letter on Company or School Letterhead stating what the ordered specimens will be used for.

Billing Information:

If we are shipping your order: You will be asked to provide a credit card to be charged when your order ships. You will receive an invoice via email prior to shipping. We will also email you a copy of your payment receipt once paid.

If you have a FEDEX or UPS account please note this on your order and we will omit the shipping cost from the quote.
